Job Summary
The KKV Weekend Case Manager/Community Support Peer provides case
management and recovery support to the community members of KKV Friday through Monday
inclusive of weekends. The KKV Case Manager will work in a multi-disciplinary team to help
unhoused individuals who are suffering from substance use and mental health issues. This will
include, but not be limited to, transportation, targeted outreach, engagement with people suffering
from co-occurring disorders, and housing disparities. The incumbent will provide case management
and peer support recovery coach services to clients under the supervision of the KKV Social
Worker and Program Director. These services include coordination of support community services
and assistance with stabilization after a period of homelessness, prevention services, self-advocacy,
development of natural supports, and the development and maintenance of community living skills.
They will focus on the stability of KKV clients and fellow team members within the KKV
community.
