Complete this application if you are requesting child support or parentage services from the Lummi Nation Child Support Program.

Complete every section that applies to your situation and provide accurate information about yourself, the children, and the other parent.

Provide Financial Information

Complete the financial worksheet when requested by program staff or when financial information is needed for establishing, reviewing, or modifying a child support obligation.

Submit the completed worksheet with any requested supporting financial documents.

• Birth certificates for the children included in the application.
• Washington driver license or state identification card.
• Tribal enrollment documentation for parents and children.
• Social Security cards.
• Existing custody orders.
• Existing child support orders.
• Parentage acknowledgments or parentage orders.
• Other relevant court orders.
• Requested financial records or income documentation.

Contact the program if a document is unavailable. Staff can explain whether other information may be accepted.

Forms submitted by email must contain the applicant’s handwritten signature before the document is scanned or uploaded.

3. You may be contacted by an Intake Specialist or assigned case manager.
4. Processing may be delayed if required information, signatures, or documents are missing.

Walk-ins are accepted during normal office hours. An Intake Specialist can help applicants understand the forms and identify supporting documents that may apply.
