The forms below may be downloaded, printed, completed, and submitted to the Lummi Nation Child Support Program.

Use This Form to Request Services

Complete this application if you are requesting child support or parentage services from the Lummi Nation Child Support Program.

Complete every section that applies to your situation and provide accurate information about yourself, the children, and the other parent.

Provide Financial Information

Complete the financial worksheet when requested by program staff or when financial information is needed for establishing, reviewing, or modifying a child support obligation.

Submit the completed worksheet with any requested supporting financial documents.

Complete, sign, and have the conditional agreement notarized, then send it to LNCSP. After LNCSP receives the agreement, reinstatement on the child support end may take up to two business days. For a suspension due to payment noncompliance, contact your assigned case manager instead.

Required for Fishers With Child Support Obligations

Under Lummi Indian Business Council Resolution #2026-109, fishers with child support obligations, including those with past-due support only, are required to sign a Conditional Tribal Fishing Validation and Child Support Compliance Agreement. One agreement is required per fisher, even if the fisher has more than one child support case.

The agreement requires timely child support payments under your controlling order or compliance with an approved payment plan. If you cannot make the required payment, contact your assigned case manager to discuss your circumstances and possible arrangements.

Important: A fishing compliance payment plan does not modify your court-ordered child support amount. If you pay less than the ordered amount, unpaid support may continue to accrue as arrears. A modification of the court order requires a separate legal process.

The agreement includes signature and notarization fields. Downloading a copy alone does not complete the signing requirement. Contact LNCSP to confirm the required execution and submission process.

• Birth certificates for the children included in the application.
• Washington driver license or state identification card.
• Tribal enrollment documentation for parents and children.
• Social Security cards.
• Existing custody orders.
• Existing child support orders.
• Parentage acknowledgments or parentage orders.
• Other relevant court orders.
• Requested financial records or income documentation.
