Complete this application if you are requesting child support or parentage services from the Lummi Nation Child Support Program.

Complete every section that applies to your situation and provide accurate information about yourself, the children, and the other parent.

A handwritten signature is required.

Financial Worksheet

Provide Financial Information

Complete the financial worksheet when requested by program staff or when financial information is needed for establishing, reviewing, or modifying a child support obligation.

Submit the completed worksheet with any requested supporting financial documents.

Provide complete and accurate information.

Missing financial information or supporting documents may delay review of your case.

Supporting Documents

Depending on your case, you may be asked to provide copies of:

  • Birth certificates for the children included in the application.
  • Washington driver license or state identification card.
  • Tribal enrollment documentation for parents and children.
  • Social Security cards.
  • Existing custody orders.
  • Existing child support orders.
  • Parentage acknowledgments or parentage orders.
  • Other relevant court orders.
  • Requested financial records or income documentation.

If you have questions about forms or need to communicate with the Child Support Program, you may email us at LNCSP@lummi-nsn.gov.

For your privacy: Please do not include Social Security numbers, bank account information, or other sensitive personal information in the body of an email. If Child Support staff request documents from you, they will provide instructions for submitting them.

Before You Submit

  • Complete every section that applies.
  • Review the form for missing information.
  • Sign the application by hand.
  • Attach copies of applicable supporting documents.
  • Keep a copy of the completed documents for your records.

What Happens After Submission?

  1. Program staff review the form and supporting documents.
  2. Staff determine whether additional information is needed.
  3. You may be contacted by an Intake Specialist or assigned case manager.
  4. Processing may be delayed if required information, signatures, or documents are missing.

Is there a fee to apply for services?
No. There is no application fee.
Can I sign the application electronically?
No. Electronic signatures are not currently accepted. The application must contain the applicant’s handwritten signature.
What if I do not have every requested document?
Contact the program if a document is unavailable. Missing documents may delay review, but staff can explain whether additional or alternative information may be accepted.
Can someone help me complete the application?
Yes. Walk-ins are accepted during normal office hours, and an Intake Specialist can help applicants understand and complete the application.
How do I submit a form by email?
Because the program does not yet have a shared public email address, call (360) 312-2235 for current email submission instructions.
