Job Summary
The Community Health RN provides professional nursing care, health
education, care coordination, and disease prevention services to individuals, families and
populations within the community and ambulatory care settings. The RN promotes health equity,
improves access to care, supports chronic disease management, and collaborates with
interdisciplinary teams and community resources to optimize patient outcomes.
